Appointment Information:
For your initial visit, please arrive fifteen minutes early to complete the necessary paperwork. If you are unable to keep an appointment, please call the office at least 24 hours in advance to cancel. We strive to run an on-time office.
Please provide us with at least one phone number by which we may contact you. If you do not confirm a scheduled appointment when you are contacted by our office, your appointment may be canceled at our discretion.
Please be prepared to pay your co-payment at the time of your visit. We accept cash, and credit cards. You must also present your insurance card at the time of your appointment.
Please call our office between 8:00 AM and 5:00 PM for all routine matters. After hours, you may reach our providers by leaving a message on our answering service. Messages will be checked every hour, on the hour. Someone will get back to you within 24 hrs. For all emergencies, please dial 911 or go to the nearest emergency room.
